Please make sure your student’s Club Permission Slip is fully completed and turned in on time so they can participate!

✅ Student signature REQUIRED ✅ Parent signature REQUIRED

Some forms have been returned due to missing signatures. Please double-check backpacks and folders to be sure everything is filled out correctly. We don’t want any student to miss out!
